I just didn’t like her in the commoner clothes or the mage robes, and I haven’t seen this on the nexus yet. If this mod already exists somewhere, then sorry ’bout that.
Installation:
Just extract the archive into your override folder.
Uninstallation:
Delete the mod files from your override folder.
Conflicts:
Will conflict with any other mods that change Flemeth’s utc files, such as Improved Atmosphere. To avoid any conflicts with mods such as these, make sure to delete the conflicting files from the other mods.
You can also place my files in the toolsetexport folder to prioritise it above other mods. If you don’t have a toolsetexport, you can just create one yourself. Any mods placed in here will override others of the same kind.
No other mods are required for this to work.

An attempt at recreating Flemeth as she appears in Dragon Age 2. She will first appear harmless, as Alistair’s dialogue implies. But should you confront her later, she will be wearing her armor and spikey hair.

This mod is my attempt to recreate Flemeth’s appearance from Dragon Age 2 in Dragon Age: Origins. It’s not a perfect match, but it’s about as close as I could get within the limitations of what I have to work with in Origins. The wrinkles, for instance, are pretty much impossible to recreate without a matching texture. I have merged my «Lore Version» Flemeth with this one, which should make Flemeth appear at first as a slightly mad old crone, since that is what Alistair’s dialogue implies. «This is a dreaded ‘Witch of the Wilds’?»
However, when you encounter her in Morrigan’s dream sequence, or when you confront her for Morrigan’s personal quest, she will appear in her true form. Also, should you decide to kill her, she will drop the staff and greatsword variant of the Staff of Parthalan (an edit I made for my main warden, I wanted a way to get it and it suits her).
Installation
Simply drag and drop into your override folder. (. Documents/Bioware/Dragon Age/packages/core/override)
Be sure to delete from your override folder any other instances of «hf_gen_flemeth.mor», «pre210cr_flemeth.utc», «pre211cr_flemeth.utc» and «cir300cr_flemeth.utc».

1. It will only take affect if you haven’t met the NPC yet, so better to start a new game.
(see HOW-TO below for use it AFTER you’ve met the NPC)
2. Flemeth only changes her clothes when you come to confront her about her identity.

Unzip and drop the folder in your override directory
Delete the override directory (although this might make the NPC invisible)
1. Locate your game save file (.das). They are under \My Documents\Bioware\Dragon Age\Character\[name]\Saves\[slot]\
3. Really, you back up the das file, right?
4. Open the das file with a HEX editor (e.g.: XVI

Flemeth is realistic face morph of DAO Origins ancient Witch of Wilds Flemeth. As I have recently made «new» and quite beutiful Morigan I felt it was necessery for me to try is to make her «new» / better (Flemeth) mother but it was easier said then done. Making of her new face has prowen as quite a bit of chalange as she must for the sake of story look as (much) older version of «her» daughter Morigan with obvious remains of her previous beauty. On top of that in my opinion her looks should show that she is an ancient abomanation / wild witch / shamaness from some bygone era. After quite few unsaticfactory blanks I did make make this version of her face which I see as satisfactory and the most appropriate for that role.
To install you will have to navigate to your BioWare game folder in your documents. (\BioWare\Dragon Age\packages\core\override) and place my Flemeth folder there. That is all you have to do. There is no need for activation or anything else. If you don’t like this new look simply delete this folder and original Flemeth will be restored to your game.
Bugs/conflicts: none known but if some other mod does same (installs new hf_gen_flemeth.mor file) it will probably overwrite previous one but for sake of your game-play take care to have only one of them.
Note on v1.2: Similar to v1.0 / v1.1but Flemeths favce somewhat younger and a bit less forbidding. She now has Morrigans (golden) eyes. There is new set of Morrigan eyes for her supplied as optional extra. These are Morrigan’s (game) eyes as well so choice for change is yours. If you want to try them copy them into your override folder / Flemeth folder there is fine. If unhappy simply delete / remove these eye-texture files from there.
Many thanks to hair modders Ren, Anco and to all others who did make use of this glorious hair in DAO possible. Thanks to BioWare for really great game.
